Willesden Junction to Gunnersbury is an uneven 4tph.
Upper Holloway to Hampstead Heath is 1tpd.
Battersea Park to Wandsworth Road is 3tpd.
I've added Willesden Junction to Gunnersbury as every 15 minutes (even though it's really every 11/19 minutes), thanks.

The other two aren't on the tube map, so (executive decision) they're not going in.
Liverpool St to Hackney Downs is pushing the definition of turn-up-and-go: depending on when you arrive, the next train could be within the next 3 minutes, or 12 minutes away.
The services between Chalfont & Latimer and Amersham, between Shepherds Bush and Clapham Junction, and between Queens Road Peckham, Peckham Rye and Denmark Hill are better than that since they are augmented by Chiltern or Southern services, and as far as I am aware tickets are fully interavailable.
Hainault - Woodford is technically true but it could be argued most people would be going to/from London and so could go either way around the loop.

In the future you would have to add Maidenhead - Reading (every 30 minutes) although other services will also run every 30 minutes. Burnham, Langley and Iver would also only have a half-hourly service - unless plans change of course. A few more stations to the west of Paddington will only have a train every 15 minutes.
There's also 1tpd from Rickmansworth to Croxley, or 2 in the other direction. But arguably not on the tube map, and definitely not very useful...
Clapham Junction <-> Highbury and Islington is also every 15 minutes. And it's on the map.
