In Hertfordshire, the Watford Fire Museum seems to be open on the third Saturday of every month, except for June and July, when it opens on the second Saturday of the month. However, I don't know if this counts, as a limited section of it is also open Wednesday mornings.

There's also the Buntingford Heritage Centre. However, this is open twice a month (albeit for a couple of hours) and may be a bit remote if you're looking for attractions near to London.

Probably the most promising candidate in Herts is the Datchworth Museum, which is only open on the third Sunday of each month from March to November. It's between Welwyn and Stevenage. I would imagine you could get to it quite easily from London via. the A1(M) or from Knebworth railway station on the line from King's Cross.

There are plenty of other good museums in Herts, but most are certainly open far more regularly than once a month.

What about regular in season openings,e.g. Mountnesing Windmill, Essex,3rd Sun May-Oct?

Red Cross Garden near Borough Market, established by Octavia Hill (co-founder of The National Trust) in 1887, has tours of the now privately-owned Bishop's Hall (which was designed by Elijah Hoole of Toynbee Hall fame) on the last Sunday afternoon of every month except December.
