I don't think I've ever seen both platforms in use at Bank at the same time: peak hour services usually use the one on the left (which saves valuable seconds as the exit is straight, rather than across the scissors crossing) but off-peak services use the one on the right.
(Running from alternate platforms would also cause more wear and tear on the points)
In my experience a great number of people, me included, mass at the far end of the platform, for anyone whose main line trains leave from the low numbered platforms at Waterloo, they will want the exit near the front of the train.
If "the minutes to departure pass", something has gone wrong - the typical turnround time at Bank is less than a minute: (remember that the next train is only two minutes behind). In the mornings, passengers alighting from the far end of the platform at Bank will see their train set off back to Waterloo long before they've reached the barrier:
how did the driver manage to fight his way against the crowd to get to the other cab so quickly? He didn't: he's still here, waiting to take the next train out.
A process known as "Stepping back", which happens at Brixton too with Victoria Line train, and Met line trains at Aldgate.
