Formerly:
West Ruislip (for Ickenham)

And,
Holborn (Kingsway)

According to http://underground-history.co.uk/downtour2.php, the disused Down Street tube station went by the name Down Street (for Mayfair).

Are they going to rename it 'North Greenwich for the North Greenwich Arena' for the duration of the Olympics? Or just North Greenwich?

Given that it is going to be called that for Olympic purposes, and clearly it is an un-fact that it is ever called anything else (see http://www.london2012.com/north-greenwich-arena)

(semi-serious question)

Kensington (Olympia)

I seem to remember that Warwick Avenue has for Little Venice...

Turnham Green (for Bedford Park)

Burnt Oak (for Watling)
Burnt Oak (for Stag Lane Aerodrome)
have been visible in my time. Stag Lane Aerodrome closed a long while ago but is still labelled on Google Maps...

What is the reason for the year long hiatus of the "for the O2" from Sep 2009 to Sep2010?

Although it is not an Underground Line you forgot to mention the best one...Bedford to St. Pancras....The Bed Pan Line!
This name for the line is actually in common use

Tooting Bec still signed as 'Trinity Road' in some places.

They always announce Old Street for Moorfields Eye Hospital. Not sure if it's signed anywhere.

Pimlico (for Tate Gallery)

Not a roundel, but the old tiled walls at Hampstead read 'HEATH STREET'.
