Canary Wharf → Heron Quays is a particularly interesting one, because in fact Heron Quays DLR station is closer to Canary Wharf Underground station than Canary Wharf DLR station is. So it should be the official interchange from Underground to DLR at Canary Wharf.
One very 'unofficial' OSI I've used many times in years past is that between Bethnal Green (BR) and Whitechapel, requiring a 6-odd minute walk between the two. Was convenient insofar as getting from NE London to S/SE avoided the need to slog it by tube and at via least one terminal through the middle - a task obviated by the Overground now, but also useful for getting from deep north London to the south without having to worry about the extra cost of a zone 1 travelcard. That convenience is diminished now as such a product is no longer available (!) in a daily format and two z23 singles costs more than a 1-3 but the introduction of an official OSI might tempt me back. I think the distance could justify it.
When there were two stations called Shepherd's Bush, didn't they share a label between them? Seems like it would be a neater way to deal with the Bethnal Green problem.
Four? Eer, no... I'd add Paddington (Praed) to Paddington (Bishops) to that list. 3m, 30s - Used to be quicker but became longer when the new H&C ticket hall opened.
(I know you mentioned it in your list of caveats, but they must be considered as two separate stations as they have separate gatelines... even though they're within the 'same station')
Leytonstone to Leytonstone High Rd is also close enough to walk in less than 10 minutes and is the only place where the Goblin meets the Central Line.

I've always wanted the 2 Bethnal Green stations and whitechapel to be considered OSI. It would mean many people could avoid zone 1 and also, as I live nearby I could cheat the system if I needed to pop home to grab something by continuing my journey from a different station that I arrived at :) I sometimes do this with the two dalston stations as there's a shop I like nearby.
I often walk between Finchley Road & Frognal (Overground) to Finchley Road (Underground) for the Metropolitan Line, about 5 mins.
What about Bank to Monument?
The Hackney Central/Hackney Downs footbridge is of course a replacement for an earlier one removed in the 50s or 60s.

On Hackney Downs/Hackney Central, was there not an old interchange by stairs in a kind of tower structure, of which at least part remains near the south east end of Hackney Downs?

Gosh, I never noticed an ugly new hotel under construction at Tower Hill. Must open eyes more.

I was interested to read that the chain concerned was looking for further development opportunities in (amongst others) noho and midtown. Pah.

Ickenham to West Ruislip is an interesting OSI as the distance is so long - over a kilometre.
The Hackney Downs to Central link now has a target completion date of July. This is from the list of milestones in the 2015/16 Budget document (see page 56 of the pdf) that goes to the TfL Board this week.
Ugh. The vile "Noho" is "north of Soho", but it already has a name: sensible people call that part of London "Fitzrovia".

Some people occasionally ask what Soho is "south of", making a false assumption based on SoHo and NoHo in New York.

"Midtown" is another marketing stupidity, lumping together St Giles, Holborn and Bloomsbury.
What complicated travelling lives are led on the other side of the great divide. I never appreciated before how simple life is here down south. We just don't have that many changes of tube/overground/train services to confuse us.😳😳

No, you just have Clock House, Kent House and Beckenham Road all within striking distance, not to mention one of the Dulwich stations is on a different line to the others, and let's not even start about which Streatham I should be heading for.

But you do have trams. Trams are cool. Except Blackhorse Lane, which I always confuse with Blackhorse Road.
Oh, and Morden Road/Morden, Waddon Marsh/Waddon and Centrale, which is... where?
RE: Dangleway, I think they should have extended it deep into North Greenwich station, crossplatformed with the Jubilee Line towards Central London and based their whole silly exhibition thing on the lonely north bank. It might have convinced people to use it more.

The spellchecker refuses to believe that Dangleway is a word. This is blasphemy of the highest degree.
In the olden days when Thamelsink was shown on the Tube map you could have counted Kings Cross St Pancras to Kings Cross Thameslink - although at some times there was a link from the Piccy platforms. However, St Pancras Thameslink is quite a trek from KXSP.
I have in the past argued against showing some of the more obscure interchanges on maps. They're fine if you know the area or are forewarned by a journey planner, but there may be any number of reasons (weather, luggage, dislike of roaming an unfamiliar area) why someone may not appreciate an unexpected walk having seen the link shown innocuously on a map.
@ AS - I understand those concerns. However they could be readily offset by the the time / financial saving of not taking a very long alternative route via Zone 1 for example. Further it cannot be beyond the capability of TfL and the local councils to create a well signposted, safe walking route and to provide appropriate detailed info to aid people in planning their journey / making the interchange. Giving people a decent choice is all that's needed and it's not hard to do.
Are there any OSIs between stations of the same name that take longer than these four?

I forgot how luminous the District line green used to look. So many things I want to say about that map but they would take us so off topic...
Hackney Downs → Hackney Central finally opened on 23rd July 2015 (and it's about 3 minutes from one end of the connected walkway to the other).
Wasn't there a connection between the Wathamstows at some time in the past, as with the Hackneys? If so, what happened to it?

I believe there was also a walkway between Bow (NLR) and Bow Road (GER) stations.
